Impact of the Canterbury earthquakes on dispensing of psychiatric medication for children and adolescents: longitudinal quantitative study

Background
Natural disasters are increasing in frequency and impact; they cause widespread disruption and adversity throughout the world. The Canterbury earthquakes of 2010–2011 were devastating for the people of Christchurch, New Zealand. It is important to understand the impact of this disaster on the mental health of children and adolescents.
AimsTo report psychiatric medication use for children and adolescents following the Canterbury earthquakes.
MethodDispensing data from community pharmacies for the medication classes antidepressants, antipsychotics, anxiolytics, sedatives/hypnotics and methylphenidate are routinely recorded in a national database. Longitudinal data are available for residents of the Canterbury District Health Board (DHB) and nationally. We compared dispensing data for children and adolescents residing in Canterbury DHB with national dispensing data to assess the impact of the Canterbury earthquakes on psychotropic prescribing for children and adolescents.
ResultsAfter longer-term trends and population adjustments are considered, a subtle adverse effect of the Canterbury earthquakes on dispensing of antidepressants was detected. However, the Canterbury earthquakes were not associated with higher dispensing rates for antipsychotics, anxiolytics, sedatives/hypnotics or methylphenidate.
ConclusionsMental disorders or psychological distress of a sufficient severity to result in treatment of children and adolescents with psychiatric medication were not substantially affected by the Canterbury earthquakes.

The National Cancer Institute of Canada Clinical Trials Group conducted a phase II trial of trimetrexate given in a daily × 5 intravenous bolus schedule every 3 weeks in patients with measurable recurrent anaplastic glioma and limited prior treatment. There were no responses in 14 evaluable patients. We conclude that trimetrexate, given as described, is not an active agent in this disease.

The Val158Met polymorphism of the catechol-O-methyltransferase (COMT) gene may be related to individual differences in cognition, likely via modulation of prefrontal dopamine catabolism. However, the available studies have yielded mixed results, possibly in part because they do not consistently account for other genes that affect cognition. We hypothesized that COMT Met allele homozygosity, which is associated with higher levels of prefrontal dopamine, would predict better executive function as measured using standard neuropsychological testing, and that other candidate genes might interact with COMT to modulate this effect. Participants were 95 healthy, right-handed adults who underwent genotyping and cognitive testing. COMT genotype predicted executive ability as measured by the Trail-Making Test, even after covarying for demographics and Apolipoprotein E (APOE), brain-derived neurotrophic factor (BDNF), and ankyrin repeat and kinase domain containing 1 (ANKK1) genotype. There was a COMT-ANKK1 interaction in which individuals having both the COMT Val allele and the ANKK1 T allele showed the poorest performance. This study suggests the heterogeneity in COMT effects reported in the literature may be due in part to gene–gene interactions that influence central dopaminergic systems. (JINS, 2011, 17, 1–7)

Background
New research is dramatically altering our understanding of the molecular mechanisms underlying neuronal communication.
AimTo elucidate the molecular mechanisms underlying the therapeutic effects of mood stabilisers.
MethodResults from integrated clinical and laboratory studies are reviewed.
ResultsChronic administration of lithium and valproate produced a striking reduction in protein kinase C (PKC) isozymes in rat frontal cortex and hippocampus. In a small study, tamoxifen (also a PKC inhibitor) had marked antimanic efficacy. Both lithium and valproate regulate the DNA binding activity of the activator protein 1 family of transcription factors. Using mRNA differential display, it was also shown that chronic administration of lithium and valproate modulates expression of several genes. An exciting finding is that of a robust elevation in the levels of the cytoprotective protein, bcl-2.
ConclusionsThe results suggest that regulation of signalling pathways may play a major part in the long-term actions of mood stabilisers. Additionally, mood stabilisers may exert underappreciated neuroprotective effects.

Summary
This chapter discusses the use of magnetic resonance spectroscopy (MRS) and functional magnetic resonance imaging (fMRI) techniques in childhood-onset anxiety disorders. It reviews brain imaging studies in various childhood-onset anxiety disorders, including obsessive-compulsive disorder (OCD), generalized anxiety disorder (GAD), panic disorder, social phobia, and post-traumatic stress disorder (PTSD), as well as suggest directions for future investigation. Functional neuroimaging has most consistently identified the caudate nucleus, ventral prefrontal cortex, and anterior cingulate as neural substrates of OCD. These regions have shown abnormal serotonergic and/or glutamatergic function. The brain regions implicated in panic disorder are consistent with the brain networks subserving fear and anxiety. The structural imaging findings suggest that functional dysfunction in the hippocampus is associated with PTSD. The studies highlight that some of the common findings in OCD and PTSD may reflect similarities in symptoms, with both disorders involving repetitive, ritualistic behaviors and intrusive thoughts.

A fundamental study of the Li insertion behavior of a series of materials consisting of a TiO2 core having MoO3 on the surface has been carried out in order to determine the influence of the shell. These TiO2-(MoO3)z materials, where (z) denotes the fraction of coverage from a partial to a double layer, range in diameter from 40-100 Å. Calculations have been done on their theoretical lithium capacity using a maximum of Li0.5TiO2 for the core, and Li1.5MoO3 at the TiO2/MoO3 interface, and they have been compared to that found experimentally. The reversible Li-insertion capacity was shown to increase from 0.34 per Ti for the pure TiO2 sample, to 0.91 Li per transition metal when the MoO3 coverage increased to one monolayer. There was only one plateau observed in the electrochemical scans for the samples showing that they function as a single-phase material making them interesting for electrodes. The redox voltage of the TiO2/Li0.5TiO2 biphasic transformation increased 60 mV from the pure TiO2 to the sample containing one monolayer of MoO3. This effect was interpreted as due to a change in TiO2 surface charge coming from an inductive effect of Ti-O-Mo bonds.

KxMnO2 exhibits a layered structure that provides attractive properties as a cathode material for secondary Li batteries. Therefore, in the present study, our attention was paid to the hydrothermal preparation of highly homogenous KxMnO2 in presence of citric acid. Out X-ray diffraction and SEM studies indicated that the final product markedly change the phase and morphology on vary the concentration of citric acid. The prepared products have also studied with the help of TGA and DTA. EDS and DCP made the chemical analysis of the compounds. The impedance data of the materials were explained in terms of the compounds formed.

Clays have been synthesized and several types of molecules have been intercalated into them to enhance their ionic conductivity. The clay has the molecular formula of Litaeniolite, Li(Mg2Li)Si4O10F2, and the inserted molecules include PEO and two varieties of sulfone, tetramethylene sulfone and ethylmethyl sulfone. These have been made in the interest of electrolytes in lithium secondary batteries in order to produce a truly solid state cell. The products have been thoroughly characterized by x-ray diffraction to verify the uptake of the molecules into the layers, thermal analysis to observe the stabilization of the intercalated molecules, along with impedance measurements to test their conductivity.

Sodium fluorophlogopite and lithium taeniolite have been synthesized by new routes for application in lithium batteries. The fluorophlogopite synthesized by a high temperature solid state reaction, was found to be non-water-swellable and unreactive towards several mono- and divalent ions. However it was found to readily undergo ion-exchange with both copper and iron ions, with concomitant swelling to a bilayer water state. This swelled material reacted readily with long chain amines and other molecules and ions behaving like a regular swellable silicate. A taeniolite precursor was synthesized by mild hydrothermal reactions, and annealed into a well crystalline layer solid, that reacted readily with organics to form ordered composites that have potential use as battery electrolytes and cathodes.

Proanthocyanidins (condensed tannins; PA) purified from the leaves of forage legumes Trifolium arvense, Lotus pedunculatus, Lotus corniculatus, Dorycnium rectum, Coronilla varia, Onobrychis viciifolia, or Hedysarum coronarium, were added to soluble lucerne (Medicago sativa) leaf protein and incubated with strained rumen fluid in vitro. Fractions were collected and frozen immediately. Denatured proteins were fractionated by sodium dodecylsulphate–polyacrylamide gel electrophoresis (SDS–PAGE), stained, and relative levels were quantified by densitometry. In the absence of PA the large subunit (LSU) of ribulose bisphosphate carboxylase (EC 4. 1. 1. 39) was susceptible to proteolysis by rumen microflora but the small subunit (SSU) resisted breakdown. PA purified from Onobrychis was added to soluble leaf protein, at PA: protein ratios between 1:1 and 1:20. The rate of proteolysis of LSU1 was significantly reduced at PA: protein ratios of 1:2 and 1:1 (P < 0.001) and the rate of digestion was reduced by between 3- and 21-fold. In separate experiments PA isolated from the range of species described was added to rumen fluid to give PA: protein ratios of 1: 5. The addition of PA significantly reduced the rate of proteolysis of LSU, when compared with PA-free control. There were only small differences between PA from different species. The inhibitory effect of PA may have been due to PA binding to the dietary protein or to the rumen proteases, interfering with the action of proteases on susceptible sites within the substrate.
